The real story is that a summary of any given patient's medical records can be insufficient information to care for the patient involved.

For that reason, our doctors will let you know if we need medical records transferred from alternate service providers, such as a doctor or hospital, that you have been to in the past. 

Similarly, you may want our medical records sent somewhere else.

• If you would like your medical records transferred to another service provider, then we will require your new service provider to email or fax us their ‘Transfer of medical Records Request Form” and we will take care of the rest.
• If you would like to transfer your medical records to our practice please click on the “Transfer of Medical Records Form” in our forms tab and submit.

We ordinarily charge an administration fee, which is reflective of our costs, for transferring our medical records to somewhere else.
